Reach us at (120) 361-2456 to schedule your child's first appointment. We offer same-day slots for urgent needs.

Bring Your Records

Bring any existing immunization records, insurance cards, and referral forms. We'll also send a digital intake form ahead of time.

Meet Your Doctor

Your first visit is unhurried — we take time to understand your child, answer your questions, and build a care plan together.

Insurance: We accept most major PPO and HMO plans. Contact our office to verify your coverage.

Self-pay and sliding-scale options are also available for uninsured families.
